A Senior Care Navigator helps older adults stay healthy and independent using a community-based approach called social geriatrics. This means looking at all the things that affect a person’s well-being — like their home, social connections, and access to services — and offering support early, before serious problems arise. The goal is to help seniors stay in their own homes longer and avoid needing extra help or more complex care.
Their work also focuses on improving seniors’ overall well-being, encouraging independence and helping them stay active in their community. The core activities of a Senior Care Navigator include:
- Helping seniors identify their needs and priorities, and encouraging them to take action.
- Taking steps in the home to improve health and support aging in place.
- Collaborating with professionals in the health and social services system, as well as community and field workers.
- Referring seniors to community, private, or public resources based on their preferences.
- Providing personalized support, when needed, to help seniors access the services they want.
- Making home visits or staying in touch by phone.
- Promoting early identification of seniors at risk of rapid health decline through a network called Seniors Pathfinders.
Seniors Pathfinders training
The Senior Care Navigator also offers Seniors Pathfinder training to interested members of the English-speaking community. The goal is to create a growing safety net around the most vulnerable seniors across the Greater Québec City region. These trained persons will then be able to identify people at risk of accelerated aging and refer them, with their consent, to the Senior Care Navigator who will then follow up with a visit or a phone call.
