Patient Navigator

Language plays a fundamental role across the entire continuum of healthcare, serving as the main channel for communicating symptoms, diagnosis, treatment planning, and following medical instructions. Clear communication—especially when delivered in a patient’s preferred language and in plain terms—is closely associated with greater safety, fewer medical errors, and improved health outcomes. 

 

Patient Navigator Network (PNN)

Patient navigation is an evidence-based approach to support populations who face barriers in accessing health and social services due to attributes such as language and culture. The patient navigator provides an opportunity to achieve better outcomes for both the patient and the professional, as well as improving access to health and social services, as well as medical information in English, for English-speakers who live both in Québec City and outside the region. 

JH Partners is proud to be part of the Patient Navigation Network (PNN), an initiative funded by the Community Health and Social Services Network (CHSSN), with support of the Secrétariat aux relations avec les Québécois d’expression anglaise (SRQEA).  The patient navigator service has existed since 2018, previously under the leadership of the CHSSN. 

 

About the Patient Navigators

JH Partners is offering a patient navigator service that targets both our local clientele, as well as people living in the regions of the North Shore, Lower North Shore, Gaspé, and Magdalen Islands travelling to Québec City for specialized medical services.  

The JH Partners’ patient navigator can assist English-speaking patients in navigating the different steps in the care pathway as well as the informational, emotional, logistical (transportation and lodging) challenges. The Patient Navigator also links patient to existing services, as needed, facilitating access for patients and communications with health care staff, and works directly with different partners of the Québec City region health and social services public system.
